Workmen's Compensation

When a workman is injured in an industrial accident, employer must:
  • Submit an Accident Report on the prescribed Form A to Ministry of Manpower (MOM) electronically within 10 days.
  • Submit a copy of Form A together with our Claim Form which may be downloaded from this website within 10 days.
  • Submit original medical certifications, hospital/medical bills, and details of injured workman average daily wage to claim for reimbursement of hospital/medical expenses and wages.
  • Arrange for injured workman with permanent incapacity to submit the prescribed application form for Permanent incapacity together with a medical report to MOM for assessment of nature and percentage of incapacity and the amount of the award.
  • File death claim report to MOM together with the Death Certificate of the workman for assessment of quantum of compensation payable.
  • Inform MS First Capital Insurance Limited upon receipt of any Common Law claim arising out of injury to their workman.
